Reporting Relationship: Housekeeping Supervisor

Primary Accountabilities:  Responsible for general cleaning of public areas, residents’ rooms, and offices.  Public areas are defined as administrative offices, lobbies, nursing stations, med rooms, consultation rooms, bathing rooms, public restrooms, corridors, and stairwells.

Major Duties:

·         Dusts and polishes all surfaces, including high and horizontal surfaces, furniture and accessories in public areas and residents’ rooms.

·         Spot cleans carpets, walls, doors, and other painted surfaces in public areas and residents’ rooms.

·         Cleans and sanitizes public restrooms and residents’ bathrooms

·         Maintains supplies in public restrooms and residents’ bathrooms

·         Follows Infection Control Policy and Procedures

·         Dusts personal belongings in place (picture frames, knick-knacks, etc.)

·         Thoroughly cleans residents’ beds as assigned

·         Empties and cleans trash cans, replaces liners

·         Dry mops, wet mops, and vacuums

·         Completes discharge cleaning as assigned

·         Complies with Precautions Policy and Procedures

·         Any other duties as assigned

Required Qualifications:

·         Language Ability:

• Ability to communicate effectively in writing and speaking the primary language of the elders.
• Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. 

·         Cognitive Demands:

• Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. 
• Ability to deal with problems involving several concrete variables in standardized situations.

Reporting Relationship: Director of Nursing

Primary Accountabilities:

To serve elders in an honorable way by working with all facility staff in developing a team approach to care for the elders by assisting with ADLs, maintaining a pleasant environment, being a friend, taking vitals, reporting abnormalities to the charge nurse, complying with the Elder Justice Act and respecting Resident Rights.

Major Duties:

·         Acknowledgement of each elder’s care plan from reviewing Kardex daily.

·         Developing routine cares with your elders and co-workers and being flexible with any changes.

·         Preforming elder cares of bathing, hygiene & grooming, oral cares, meals, safe transfers, mobility, toileting, and any other cares described in elder’s Kardex.

·         Provides peri care and catheter care, handling and placing tubing appropriate to avoid infections.

·         Follow the facility Infection Control Program.

·         Assures all assigned elders are dressed in clothing that is clean, in good repair, and appropriate for season and time of day according to elder’s choice and preference. Changing soiled clothing upon discovery.

·         Assist elders with application of hearing aide, clean eyeglasses and any prostheses.

·         Obtains vital signs and documents accurately: height & weight, respirations, pulse & pulse oximetry, temperature, blood pressure, reports of pain, and reports of any abnormality.

·         Completes delegated task from the charge nurse

·         Use care planned assistive devices with elder.

·         Assist elder with correct diet and snacks.

·         Wear full uniform daily of name tag, pager, and walkie.

·         Answering call lights in a timely manner.

·         Immediately reports any elder grievances to the charge nurse.

·         Attending all annual required education and training provided by the facility.

Required Qualifications:

·         Must have a current and valid Kansas Nursing Assistant Certification.

·         Ability to read and understand Policies and Procedures.

·         Safe handling of equipment by attending all training prior to using equipment and following safety guidelines.

·         Ability to listen and communicate step by step procedures to elders with maintaining elder dignity.

·         Must be familiar with policies and procedures that pertain to elder care. Must promote the highest quality of care by being involved in at least one of the following: QAPI, Safety Committee, At Risk Committee, Infection Control committee, any PIPs, and on-going education and training. 

·         Ability to read and write for communication and documentation.

·         Ability to add, subtract, and understand decimals in all units of measure.

·         Compassionate, mature, sympathetic, and professional at all times.    

·         The ability to understand and carry out instructions furnished in written, oral, or diagram form.

·         Ability to learn the electronic medical records, documentation, and reading the Kardex (care plan)

·         Ability to operate a computer at a moderate skill level.

Reporting Relationship: Director of Nursing

 Primary Accountabilities:

 To serve elders in an honorable way by meeting their needs through the development and utilization of nursing staff. Ensuring the highest quality of care can be maintained at all times and properly utilizing resources throughout the facility and making independent decision in meeting these responsibilities.

Major Duties:

·         Utilize PCC throughout the shift to identify elders change in conditions.

·         Conduct proper and thorough data collection or assessments on elders.

·         Ensure all UDAs are completed your shift.

·         Ensure all documentation is completed in a timely manner.

·         Foresee needed interventions to be proactive with falls, wounds, infections, weight loss, pain, contractures, behaviors, depression, isolation, and boredom.

·         Coordinate and conduct admissions, transfers, and discharges.

·         Monitor all elders through- out the shift.

·         Review medication orders for unnecessary medications, drug to drug interaction, adverse reactions, and proper response.

·         Ensure all medications are monitored appropriately with vital signs, labs, or additional documentation.

·         Must document behaviors and implement non-pharmacological interventions prior to giving prn medications.

·         Ensure all medications have a diagnose and query physicians for proper diagnose if needed.

·         Identify issues or emergencies and responding in a calm and efficient manner.

·         Read and sign the CMA job description.

·         Initiate 48-hour base line care and update care plan as needed.

·         Provide compassionate care and assistance to elders and families.

·         Direct the day-to-day functions of the nursing activities in accordance with current rules, regulations, and guidelines that govern the long-term care facility.

·         Monitor for survey compliance and participate in the development, maintenance, and implementation of the facility’s QAPI program for the nursing department.

 Major Duties Continued:

·         Make daily compliance rounds to ensure all nursing personnel are performing their work assignments in accordance with acceptable nursing standards. Report any needed findings to the director of nursing.

·         Mentor and training of new staff and provide support and guidance to all staff members.

·         Continue ongoing training on dementia, behaviors, wounds, antipsychotic medications and leadership skills.

·         Coordinate daily administrative duties, schedule, nursing assignments and elder care.

·         Supervise all nursing staff to ensure high quality of care is provided to all elders.

·         Ensure proper utilization of any device or equipment used in nursing.

Required Qualifications:

·         Graduate from an accredited nursing school or a nursing program from an accredited college or university.

·         Current valid license as an L.P.N or R.N. in Kansas with no restrictions.

·         Current CPR certification.

·         Knowledgeable of nursing and medical practices and procedures, laws, regulations, and guidelines that pertains to nurse practice act and long-term care.

·         Must possess and demonstrate a high degree of leadership, organizational ability, communication skills, and be highly motivated to provide high quality of cares.

·         Knowledge of nurse practice act, policy and procedures that apply to long-term care.

·         Has strong organizational and time management skills and the ability to prioritize responsibilities.

·         Ability to define problems, collect data, establish facts, and draw valid conclusions.

·         Ability to apply mathematical concepts as fractions, division, multiplication, decimals, and formats.

·         Must be able to work overtime. Must be available for rotation on call as per facility protocol. Respond when the director of nursing reaches out to you by phone as appropriate.
